Non-objecting admission form
Interestingly I could not find a sensible reference for "Sec. 33.25, M.H. Law" because 33.25 of the Mental Hygiene law pertains to handling reports of abuse. Section 33 broadly refers to patients rights, hence the statement of the form which reads:
"You have been admitted to this school for the mentally retarded as a non-objecting resident. You will live, sleep, work, and play here unless you are allowed to go somewhere else.
At any time, you may tell the director of other staff members that you want to leave. However, you may not leave for three days unless the director lets you. If the director thinks you need to stay, he may ask a court for an order to keep you here.
You, your relatives, and your friends should feel free to ask members of the school staff about your condition, your status and rights, and the rules and regulations of the school."
Sounds more like a jail than a school, especially considering this patient had been admitted in 1949, and signed this in 1975.

I've seen similar medical records on other abandoned-places sites, and it just amazes me that such things are left unsecured.
